Job Title: Licensed Practical Nurse (LPN) – Home Health

• Location: Bridgeport, Connecticut
• Schedule: 40 hours per week
• Hours Guarantee: 40 hours/week
• EMR: HCHB (Homecare Homebase)

Travel Pay: $2,000/Week - $2,100/Week

Local Pay: $35-$42/Hr on W2

Required Qualifications

• Active Connecticut LPN license .
• Previous Home Health experience required .
• Experience with HCHB/Homecare Homebase required .
• OASIS experience preferred/required as applicable to the assignment.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently in a home-health environment.
• Reliable transportation and ability to travel throughout the assigned service area.
