Hudson Hill Center for Rehabilitation and Nursing MDS Assessor is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations. This role ensures accurate Minimum Data Set (MDS) assessments, Care Area Assessments (CAAs), and interdisciplinary care planning to support regulatory compliance, clinical quality measures, and appropriate reimbursement.

The MDS Assessor plays a critical role in ensuring documentation integrity, case mix optimization, and overall quality of resident care.

Location is 65 Ashburton Ave, Yonkers, NY

MDS Assessor

• Complete and submit MDS assessments (OBRA, Medicare PPS, IPA, Significant Change, etc.) in compliance with CMS guidelines.
• Coordinate and facilitate the RAI process across all disciplines.
• Ensure timely transmission of MDS assessments to CMS.
• Maintain accurate MDS scheduling and tracking systems.

Care Planning

• Lead or coordinate interdisciplinary care plan meetings.
• Develop individualized, resident-centered care plans based on MDS findings.
• Ensure Care Area Assessments (CAAs) are completed and documented appropriately.
• Update care plans based on significant change assessments.

Regulatory Compliance

• Maintain compliance with CMS, state, and facility policies.
• Stay current with RAI Manual updates and regulatory changes.
• Participate in survey readiness and respond to survey inquiries related to MDS and care planning.
• Ensure documentation supports coding accuracy and reimbursement.

Reimbursement & Case Mix Management

• Accurately capture clinical data impacting PDPM reimbursement.
• Collaborate with billing and finance departments to ensure appropriate revenue capture.
• Review supporting documentation to validate MDS coding.
• Identify and correct discrepancies in documentation.

Quality Improvement

• Monitor Quality Measures (QMs) and Quality Reporting Program (QRP) indicators.
• Assist in developing performance improvement plans.
• Provide education to staff regarding documentation and assessment standards.

Interdisciplinary Collaboration

• Work closely with Nursing, Therapy, Social Services, Dietary, and Medical Providers.
• Provide education and guidance on documentation requirements.
• Communicate effectively with residents and families regarding care plans when appropriate.

Qualifications

Education

• Current RN license required (preferred in most facilities).

Experience

• Minimum 1-2 years of experience in long-term care or skilled nursing.
• 1 years MDS experience required
• Strong knowledge of CMS RAI Manual and PDPM.

Certifications

• RAC-CT (Resident Assessment Coordinator-Certified)
• CPR certification

Required Skills & Competencies

• Strong understanding of PDPM reimbursement methodology.
• Detail-oriented with high documentation accuracy.
• Knowledge of ICD-10 coding related to MDS.
• Ability to analyze clinical documentation.
• Strong organizational and time-management skills.
• Proficiency in electronic health record (EHR) systems.
• Effective leadership and interdisciplinary coordination skills.
