In today’s ever-changing healthcare marketplace, HDSW offers person-centered, trauma-informed, culturally competent services to over 3000 people each year, through a unique blend of innovative inter-related programs. Position Overview To assist clients living in the community to link to necessary medical, behavioral health, housing, social, employment, educational, and other services essential to meeting basic life needs. Sign-on Bonus $3000 paid out over the first year of satisfactory employment.
