Onsite Per Diem RN Case Manager Liaison - Discharge Planning

Kaiser Permanente’s Bellevue site seeks a Per Diem RN Case Manager Liaison for inpatient discharge planning onsite at Overlake. Working 8AM-4:30PM with one weekend per month, you will rotate between care settings and coordinate discharges for KFHPW patients.
You will collaborate with primary and specialty teams to ensure adherence to medical plans, optimize functional status, and smooth transitions to home or facilities.
