Summary:

Responsible for direct patient care and case management of Hospice patients in outpatient settings. Ensures that end-of-life care is delivered within quality and utilization guidelines and is directed toward the achievement of desired clinical outcomes for patients and their families.

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)

Qualifications:

• Associate Degree in Nursing

• Registered Nurse Licensure - Compact/Multi

• Minimum one year in direct patient care setting or home health care.

• Valid Driver's License 

• BLS required at time of hire
