Medical Social Worker (MSW) – Acute Care Hospital

Location: Torrance, California

Duration - 13 Weeks

Position Summary

We are seeking an experienced Medical Social Worker (MSW) to join a dynamic Acute Care Hospital team. The Medical Social Worker will provide comprehensive psychosocial support, discharge planning, care coordination, and resource management for patients and their families. This role requires strong collaboration with physicians, nurses, case managers, and community agencies to ensure safe and effective transitions of care.

The ideal candidate will have recent acute care hospital experience, exceptional communication skills, and the ability to manage a high-volume caseload in a fast-paced healthcare environment.

Key Responsibilities

• Conduct psychosocial assessments to evaluate patients' emotional, social, financial, and environmental needs.
• Develop and coordinate comprehensive discharge plans to support safe patient transitions.
• Collaborate with physicians, nursing staff, case managers, rehabilitation teams, and community organizations.
• Provide counseling, crisis intervention, emotional support, and patient/family education.
• Identify barriers to discharge and implement solutions to facilitate timely transitions.
• Coordinate post-acute services including home health, skilled nursing facilities, rehabilitation centers, hospice, and community support programs.
• Advocate for patients and families regarding healthcare access and community resources.
• Participate in interdisciplinary care team meetings and discharge planning rounds.
• Maintain accurate and timely documentation in accordance with facility and regulatory standards.
• Assist with utilization management and care coordination activities as needed.
• Provide weekend and holiday coverage according to department needs.

Required Qualifications

• Master's Degree in Social Work (MSW) from an accredited institution.
• Minimum of 2 years of Acute Care Hospital Case Management or Medical Social Work experience .
• Strong background in discharge planning and care coordination.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and manage a high-volume caseload.
• Knowledge of community resources, healthcare regulations, and post-acute care services.

Preferred Qualifications

• California Social Work license or eligibility preferred.
• Experience working within large acute care hospital systems.
• Familiarity with utilization review and multidisciplinary care coordination.
