Hospice RN Case Manager - Home Care & Growth

Job Description Job Description Summary We are hiring a Hospice RN Case Manager for a full-time, permanent opportunity in Denton County, TX . This position is ideal for compassionate Registered Nurses with hospice experience who are dedicated to providing exceptional end-of-life care to patients in their homes while supporting families throughout the hospice journey. Location Denton County, TX Salary $85,000 – $95,000 annually Additional Compensation Mileage Reimbursement: $0.65 per mile Cell Phone Reimbursement: $60/month Schedule Full-Time Monday–Friday On-call rotation required Travel throughout assigned territory Key Responsibilities Provide skilled nursing care based on individualized care plans. Perform comprehensive patient assessments and monitor changes in condition. Administer medications and treatments as prescribed. Educate patients and families on hospice philosophy and symptom management. Coordinate care with physicians and the interdisciplinary hospice team. Maintain accurate and timely EMR documentation. Respond to urgent patient needs and provide crisis intervention. Ensure compliance with hospice regulations and quality standards. Requirements Active Texas Registered Nurse (RN) License. Minimum 1 year of Hospice nursing experience. Minimum 1 year of Case Management experience. Valid driver's license, reliable transportation, and current auto insurance. Strong communication and patient education skills. Experience with EMR systems. WellSky (Consolo) experience preferred.
