- Part-time, flexible schedule
- Base pay + commission
- Mileage reimbursement
- Opportunity for growth
- Build relationships with healthcare professionals
- Visit facilities and introduce our services
- Develop referral partnerships and follow up
- Track outreach and referrals
- 5+ years of experience in social work, discharge planning, or a related field
- Knowledge of Omaha’s healthcare or senior care community preferred
- Strong communication and networking skills
- Self-motivated with reliable transportation
