This is a 13-week contract position.

Schedule

Day 4x8-Hour shift (08:00 - 16:30)

Job Description

We are seeking a qualified Home Health Speech-Language Pathologist (SLP) to provide exceptional care to patients in their homes. This position requires an active Oregon license and entails visiting up to 6 patients per day with a focus on achieving high productivity standards.

Responsibilities

Conduct evaluations and assessments for speech and language disorders.
Develop and implement individualized treatment plans.
Provide patient and family education regarding communication and swallowing disorders.
Optional: Participate in interdisciplinary team meetings to discuss patient progress.

Required Experience / Certifications / Licensure

Minimum 1 year of skilled home health experience required.
Oregon Speech-Language Pathologist license required.
BCLS certification required.
Familiarity with Oasis documentation is a plus.
