In-Office Home Health Case Manager — Care Coordination

Tricare Home Health Services Inc

Tricare Home Health Services Inc in San Antonio, TX, is seeking a full-time in-office Case Manager to coordinate patient care across the city. You’ll work with field nurses, physicians, and the office to keep everyone aligned and your caseload moving smoothly. The role includes AI-assisted documentation tools and a modern EMR.
