Ambulatory Care Pharmacist job at busy outpatient clinic in Pennsylvania.
Monday - Friday, day-shift position Am Care duties:- Direct care of patients
- Education of physicians and staff regarding clinical information
- Implementation of clinical programs that involve patients/ drug therapy.
- Responsible for managing the selection and utilization of pharmaceuticals and supports core clinical programs such as DUR, DIS and formulary management.
Great opportunity to work at a integrated health system in Pennsylvania. Ideal candidate: BS in Pharmacy or PharmD. Prior Ambulatory Care experience or PGYI. Active PA pharmacist license or ability to obtain. Relocation assistance available.
