Job Title: Advanced Practice Provider (Senior Care) – Nurse Practitioner/Physician Assistant Part-Time, 12-20 hours per week | High-Compensation Opportunity Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship-centered care at premier Senior Care communities in Sugar Hill and Grayson, GA. This part-time role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity. What You’ll Do Provide compassionate, relationship-based care to older adults in an assisted living environment. Build trust with patients, families, and facility staff through clear communication and consistent presence. Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient. Collaborate with the facility’s care team for seamless, coordinated support. Apply home health or hospice experience to improve outcomes for medically complex patients. Maintain flexible availability for patient visits and on-site presence. Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary). Master’s degree from an accredited program. Minimum of 3 years of clinical experience, preferably in geriatric, hospice, or home health. Active DEA license (or eligibility to obtain). Availability to work on-site in Sugar Hill and Grayson; occasional visits to additional locations may be required.
