As a member of the Optum Health Enterprise Clinical Services team, you'll be an integral part of our vision to make healthcare better for everyone.The Licensed Social Worker serves as a key member of the interdisciplinary healthcare team, managing non-HCBS waiver member care to improve clinical outcomes, financial performance, and member satisfaction. This role oversees the member's case within the scope of licensure, provides guidance to non-RN clinicians, and develops, monitors, and adjusts individualized care plans to ensure optimal health across the care continuum.This position follows a hybrid work from home (WFH) model, with field visits as needed. The schedule is Monday - Friday, 8:00 AM MT - 5:00 PM MT, no evenings, weekends or holidays.Must reside and hold an unrestricted license in the state of Texas.We are committed to your well-being and growth, offering a comprehensive package of perks and benefits with varying eligibility based on role, including:Competitive salaryFlexible work models & paid time off, when you need itHealth and well-being benefits like health insurance, 401k matching, and other family support and wellness resourcesProfessional development with tuition reimbursement and dedicated learning time to advance your careerMileage reimbursementPrimary Responsibilities:Conduct comprehensive assessments (telephonic or in-person) to identify physical, behavioral, and social needsDevelop, implement, and monitor individualized care plans in collaboration with members, families, and providersCoordinate care for members with chronic conditions, disabilities, or high-risk complicationsFacilitate referrals to social services, community resources, and support programsEducate members and families on care options and available servicesServe as the primary point of contact for care coordination and advocate for member needsEnsure timely documentation of assessments, care plans, and service deliveryParticipate in interdisciplinary team meetings and support safe transitions of careRequired Qualifications:Master's degree in social work (MSW) and current Texas licensure as a Licensed Social Worker2+ years of experience in community health, case management, or clinical care settingsExperience with STAR + PLUS Texas Medicaid managed care programProficiency with MS Word, Excel, and OutlookBilingual (Spanish)Dedicated home office space and access to install secure high-speed internet in home (Cable Broadband, DSL, or Fiber)Ability to travel within assigned region; valid driver's license and reliable transportationPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 - $107,400 annually based on full-time employment.
