Overview Using a one to one and group person-centered counseling model, Community Oriented Recovery and Empowerment (CORE) program seeks to improve the functioning of individuals living with mental health diagnoses, histories of chemical and alcohol dependence, or physical disabilities, helping them achieve personal goals in the areas of recovery, employment or independent functioning. Locations Brooklyn, NY 11201, USA Description Compensation Range: $65,000 - $70,000 yearly commensurate with experience. Responsibilities CPST Services: Provide time-limited, goal-directed interventions included in the ISP with the aim of stability and functional improvement. Conducts Person-Centered Assessment, Service Planning, Documentation, and Collaboration; provide Individual and Family Psychotherapy and Counseling and Integrated Co-Occurring Disorder Treatment; actively engages around Crisis Prevention; offer customized interventions for Intensive Rehabilitation counseling to help individuals attain and sustain meaningful life roles. PSR Services: Engage in person-centered planning process and support skill building and psychoeducation to develop coping strategies, prevent crises, and promote recovery. Provide Rehabilitation Counseling to improve functioning in living, working, learning, and social environments. Maintain a caseload of clients, providing 25 hours of clinical care weekly on average. Assist clients in articulating personal goals using client-centered approaches. Collaborate with professionals across disciplines and care teams (e.g., housing specialists, attorneys, rehabilitation counselors, workforce development professionals) and conduct intake assessment, counseling, advocacy, and referral services to HARP members. Opportunity to receive training in evidence-based modalities. Hybrid work format with both remote and in-person components; majority of work in community settings with clients. Provide services as outlined in each participant’s Plan of Care and assist clients with referrals to other services and agencies as needed. Collaborate with clients’ care teams to determine steps toward goals and self-directed recovery; teach clients to identify and combat negative self-talk and fears. Support clients’ vocational/educational choices and assist them in aligning opportunities with strengths, including interview tips. Maintain case records and progress notes in compliance with state regulatory requirements; hold case conferences as needed. Other duties as assigned. Master’s Degree in the Healing Arts with active license in one of the following: LMSW, LMFT, LMHC, or LCAT. Experience with serving people with mental health diagnoses or substance use preferred. Qualifications Must have one of the following active licenses: LMSW, LMFT, LMHC, or LCAT. Master’s degree in the Healing Arts or related field.
