Hospice Admissions RN — Field-Based Care Navigator
CHESAPEAKE HEALTH PARTNERS INC
CHESAPEAKE HEALTH PARTNERS INC is seeking an Admission Registered Nurse to lead initial patient assessments and education for hospice services. You will establish plans of care with families and coordinate with the interdisciplinary team across home, facility, and hospital settings. The role requires Maryland RN licensure and at least 2 years in med/surg or home health, with hospice experience preferred.
