This position also provides call issue escalation for CCHP members or providers, may manage a case load of members with low to moderate health complexity or may provide support for Team Lead duties, during the Team Lead’s absence. **Education & Experience*** High school diploma or equivalent required. Bachelor of Social Work preferred.* Five years’ experience in a managed care or medical office/hospital environment.* **Bi-lingual (English/Spanish) Required**. Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire. **Knowledge, Skills & Abilities*** Superior verbal and written communication including motivational interviewing and adult learning principles.* Familiarity with basic medical terminology and ICD-9 and CPT coding.* Personal computer skills. Expertise with various software.applications/databases (i.e., Microsoft Office).* Highly organized, ability to prioritize, and demonstrate complex problem solving skills.* Ability to work autonomously with minimal supervision.* Demonstrated care coordination concepts. **Licensure, Registration, and/or Certification:*** Licensed Bachelor Social Work preferred.* Certified Community Health Worker required.
