MDS Clinical Nurse Supervisor (RN IV)

MDS Clinical Nurse Supervisor (RN IV) (260003NE) The Massachusetts Veterans Home (MVH) at Holyoke is seeking an experienced, collaborative and dynamic individual to serve as MDS Clinical Nurse Supervisor. The candidate will be a key member of the nursing team in managing the overall MDS process. A strong background in nursing with experience in long‑term care is essential. The role requires excellent clinical skills, attention to detail, and the ability to work independently while ensuring adherence to healthcare regulations and standards. Duties and Responsibilities Collaborate and communicate with appropriate departments to ensure that all sections of the MDS and corresponding assessments are completed by the reference date. Maintain the Facility Matrix (CMS 802) accurately and up to date. Educate staff on the MDS process, conduct audits, monitor compliance, collect data and submit reports. Coordinate the MDS schedule and collaborate with nursing and social work regarding the planning and execution to the nursing team. Serve as an MDS resource for all departments involved with the resident assessment instruments process. Apply working knowledge of PPS, appropriate combining of assessments and PDPM including the ability to train others. Educate and train new RNs on the MDS process. Educate ancillary departments and providers of documentation for MDS accuracy. Complete audits on the MDS process, related assessments and quarterly summaries for all shifts. Transmit all data in the format specified by the VA and CMS within regulations. Assist with admissions/readmissions of veterans when assigned. Assist nursing and social work with the scheduling and planning of IDC meetings, including notification of participants. Oversee compliance regarding the completion and documentation of quarterly summaries and significant changes and all associated assessment tools, limited to the Norton Scale, Fall Risk Assessments, Smoking and Wander Risk Assessments. Coordinate, develop and provide in‑service education programs. Maintain Quality Assurance/Performance Improvement data and report at least quarterly. Keep the IDT abreast of changes in the RAI manual affecting departments and documentation. Provide reports and recommendations to leadership. Complete employee performance appraisals (EPRS) on direct reporting staff. Provide direct supervision of reporting staff. Serve as a member of appropriate MVH clinical committees as needed. Continuing professional growth by attending workshops, seminars, work‑related courses, conferences and staff meetings to maintain professional proficiencies and/or licensure. Perform all other related duties as assigned. Schedule This is a full‑time (40 hours/week) position working Monday through Friday, 7:00 am – 3:30 pm. As a 24‑hour/7‑day long‑term care facility, the MVH at Holyoke may require this individual to be present during non‑traditional hours when operational needs dictate. Qualifications Maintain current knowledge of changes in MDS and regulations affecting nursing practice. Experience with MDS compliance, including CMS standards. Ability to teach and mentor others. Manage conflict and collaborate with others across the multidisciplinary care team. Leadership skills including effective verbal and nonverbal professional communication, critical thinking, conflict management and effective decision making. Explain laws, rules, regulations, policies, procedures, specifications, standards and guidelines governing agency operations and assigned unit activities. Maintain accurate records. Read and interpret documents such as physicians' orders and treatment plans. Comprehensive understanding of VA and MA regulations regarding long‑term care. License and/or Certification RAC‑CT with at least one year of experience in MDS. Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing. Current CPR certification. Minimum Entrance Requirements Applicants must have at least (A) two years of full‑time, or equivalent part‑time, experience as a registered nurse in a recognized hospital, clinic or medical facility and (B) of which at least one year must have been in a supervisory, administrative or managerial capacity or (C) any equivalent combination of the required experience and the substitutions below. Substitutions: I. A graduate degree with a major in Nursing may substitute for a maximum of one year of the required (A) experience. No substitutions will be allowed for the required (B) experience. Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed. Special Requirements for All Levels in Series Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
