Under the supervision of the Program Manager, the Case Manager meets the bio-psychosocial needs of clients by providing indicated behavioral health services, case management. The Case Manager works with parents and caretakers to assist them in accessing appropriate support services and entitlements, medical care, community resources and family support systems in an effort to facilitate family well-being.

In coordination with clinical staff the Case Manager will provide the following:

  • Works collaboratively with providers and community agencies to facilitate access to mental health, medical, and support services for enrolled families.
  • Supports the clinical intake process by conducting collateral interviews and assisting in the completion of outcome measures within expected deadlines to assess family needs and barriers.
  • Develops and implements individualized case management service plans in coordination with the program therapist.
  • Provides referrals, coordination, and linkage to services such as primary care, educational support, family resources, and rehabilitation services throughout the time the family is enrolled in the program, providing regular check-ins with families at least once every 1-2 months to provide on-going support.
  • Conducts outreach to engage newly referred families and those with inconsistent participation or emerging needs.
  • Participates in community events, coalitions, and stakeholder meetings to promote program services and strengthen referral networks.
  • Gathers and shares relevant community resources to support the evolving needs of program families.
  • Provides home- and community-based behavioral coaching in collaboration with therapists to support parenting skills and strengthen the caregiver-child relationship.
  • Assists clinical team in delivering group-based services (on-site, virtual, or community-based) that promote attachment, skill building, and family well-being.
  • Supports family engagement activities such as phone screenings, parent group enrollment, and program announcements.
  • Helps families connect to a medical home and communicate with primary care providers to support coordinated care.
  • Maintains accurate documentation of service coordination and outreach activities in the medical record.
  • Will seek and attend opportunities for outreach to promote PLF services in the community (e.g. schools, libraries, community parks, conferences) by conducting presentations and tabling.

EDUCATION AND EXPERIENCE:

  • Associate's degree in psychology, sociology or related field required. Bachelor's degree preferred.
  • Ability to read, write and speak both English and Spanish fluently required.
  • Previous experience working with families, children, or in behavioral health or social services is preferred.
  • Knowledge of case management principles; the impact of trauma (e.g., domestic violence, child abuse, marital problems) and how it affects the development of healthy family relationships; methods used to facilitate parental skills to enhance family relationships; cultural competencies relevant to serving the target population; ethics and standards relevant to client/staff interactions and child abuse reporting; best practices in case/client file documentation.
  • Experience in effective teamwork and collaboration with co-workers, preschool program staff, social services agencies, and other community providers.
  • Proficiency in Microsoft Office (including Word), database interfaces, Internet, data entry, PowerPoint, and email.
  • Ability to organize and set priorities, exercise sound independent judgment, and navigate modern technology (e.g., desktop computers, mobile phones, laptops, web-based systems, PDF tools).
  • Strong communication skills, including active and reflective listening, social perceptiveness, and the ability to write and speak coherently and effectively.
  • Good filing and organizational skills, excellent phone etiquette, and ability to quickly learn unfamiliar technologies.
  • Ability to work with a diverse group of clients, staff, and community representatives.

OTHER/SPECIAL:

  • Must pass criminal background check, including Live Scan fingerprinting and Sex Offender Registry check.
  • Valid California driver's license and automobile insurance required for personal car usage; MUST have access to reliable transportation.
  • Must have CPR certification or be willing to acquire on the job.
  • Must have medical clearance certifying absence of TB. Free from debarment.
  • Ability to drive to different off-site related tasks and projects.
