Base pay range $37.00/hr - $40.00/hr Direct message the job poster from Medix Contract : 9 Months (with potential for extension) Pay : $37–$40/hour Shift Start Times : Vary between 5:00 AM – 12:00 PM (Day Shifts Only but must be open to any start time) Location : Remote (Must reside in a Compact State or have CA RN License) Required: Compact license - BSN required Preferred: Chronic disease management experience in outpatient settings (not required!) Key Responsibilities Act as a care guide and advocate for patients navigating the healthcare system Collaborate in patient-centered care team meetings and quality improvement initiatives Deliver health and disease education and promote self-management practices Support care coordination with internal teams and external healthcare organizations Monitor post-hospitalization transitions (admissions, discharges, ER visits) Manage care for high-risk patients with chronic conditions and multiple co-morbidities Participate in preventive screenings and assist with day-to-day patient interactions Promote clear communication among care teams and providers Assist with medication management and refills under protocol guidance Support clinical audits, reporting, and evaluation of Virtual Care programs Collect and analyze biometric and health data for population health initiatives Provide clinical support and guidance to Medical Assistants and other care staff Qualifications Required Licensure : Active RN license in a compact state or California (CA) license Compact RNs must reside in a compact state Willingness to obtain additional licensure as needed (facilitated by employer) Education : Bachelor of Science in Nursing (BSN) required Experience : 2–5 years in a clinical setting, ideally in outpatient care with chronic disease management Experience using Electronic Health Record (EHR) systems Proficient with Google Suite and other basic computer tools Additional Seniority level: Associate Employment type: Contract Job function: Health Care Provider Industries: Hospitals and Health Care and Medical Practices #J-18808-Ljbffr
