Home Health Physical Therapist
Pride Health is hiring a Home Health Physical Therapist to support our clients home health patient population across Wabsh, Huntington, and Grant County, IN. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
Location: Marion, IN 46952
Coverage Area: Wabsh/Huntington/Grant County
May also support patients in Blackford, Delaware, and Madison Counties (as needed)
Job Type: Contract
Shift: Monday–Friday 8:00 am to 4:30 pm (40 hrs/week)
Weekend Rotation: Every 3rd Saturday
Pay range: $54 to $56/hr on W2
Mileage reimbursement provided (from first patient to last patient visit)
Key Responsibilities:
- Conduct comprehensive patient evaluations and develop individualized physical therapy plans of care in accordance with state guidelines
- Perform home health visits, including:
- Start of Care (SOC)
- Resumption of Care (ROC)
- Evaluations
- Discharges
- Deliver skilled physical therapy interventions to patients in their homes
- Complete all clinical documentation within 24 hours of each visit
- Participate in interdisciplinary team meetings, case conferences, and branch meetings
- Maintain flexibility in scheduling to meet patient care needs
- Participate in weekend rotation and be available for occasional holiday coverage
Required Qualifications:
- Current, unencumbered Physical Therapy license in the state of Indiana
- AHA BLS certification required
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Previous home health experience with understanding of Medicare guidelines
- HomeCare HomeBase (HCHB) software experience preferred.
