Role Snapshot Role: Nurse Practitioner Location: Saginaw, MI • Onsite at skilled nursing & long-term care facilities Pay: $123,000–$135,000 base + corporate bonus Schedule: Mon–Fri, days • No on-call • No weekends Team: Skilled nursing & long-term care partners What You’ll Do Provide care in nursing and long-term care facilities Manage chronic conditions and perform full assessments Close care gaps (HEDIS and STAR measures) Lead care planning with facility teams Support and educate patients’ families Must-Haves Active, unrestricted APN license in Michigan Board certified and Medicare credentialed 6+ months in skilled nursing or long-term care Solid with digital tools and team-based care Able to work onsite in Saginaw, MI Perks & Pay Pay: $123,000–$135,000 base + corporate bonus Insurance: 90% medical/dental/vision (70% for dependents) Other: $10K life insurance, HSA/FSA, 401(k) match Time off: Up to 160 hours PTO + 10 holidays Growth: Paid CE days, stipend, full learning tools Schedule & Setup Mon–Fri, daytime hours No weekends, no call Onsite at partner sites in Saginaw, MI Impact & Growth Your work helps patients stay healthier, safer, and out of the hospital.
