Registered Nurse – Transitional Care Coordinator Yale New Haven Health – Trumbull, CT Overview Assist the organization in living its Vision, Mission, and Values (integrity, patient‑centered, respect, accountability, compassion) while working under the supervision of the Nurse Manager/Clinical Coordinator. Responsibilities Assesses the patient’s physical and psychosocial condition continuously throughout the continuum of care. Develops, communicates, and documents an individualized plan of care reflecting patient problems and interventions to achieve desired outcomes, involving patient, family, and health‑team. Implements and coordinates the plan of care in collaboration with the health‑team. Evaluates the effectiveness of the plan, patient response to care, and readiness for discharge. Revises the plan as needed and makes referrals where indicated. Plans education based on assessment of patient and family learning needs and readiness. Provides education and documents outcomes. Serves as preceptor for new staff. Functions as charge nurse, delegating and supervising unit activities. Meets performance expectations for customer service, teamwork, resource utilization, staff and self‑development as outlined in performance review. Performs other duties as assigned or directed to ensure smooth operation of the department/unit. Demonstrates competency in caring for neuro patients, including airway management, tracheostomy care, and assisting physicians during neuro procedures. Performs pharyngeal, endotracheal, and tracheostomy suctioning according to established policy and procedure. Uses equipment and cares for patients with a halo brace. Proficient in using computer software, including electronic patient records. Displays ability to use the internet for literature searches regarding patient care.
