Medical Director

We are seeking a dedicated full-time Medical Director to lead our innovative Program of All-Inclusive Care for the Elderly. This leadership role offers an exciting opportunity to provide comprehensive medical oversight and quality care coordination for seniors with chronic care needs. The goal is to help seniors maintain independence while living safely in their communities.

The Program serves individuals aged 55 or older, certified to need nursing home care, who are able to live safely in the community at the time of enrollment. While all participants must be certified to need nursing home care to enroll, only about 7 percent nationally reside in a nursing home. If a participant needs nursing home care, the program pays for it and continues to coordinate the participant's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. The program provides the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their homes for as long as possible.

The Medical Director provides general medical direction and supervision of the medical aspects of participant care. They are responsible for the delivery of participant care, clinical outcomes, and oversight of the Quality Assurance Performance Improvement (QAPI) program. The Medical Director will develop and maintain medical policies, procedures, standards, and protocols. They will provide direction and supervision to Primary Care Providers and Advanced Practice Providers.

The role involves conducting history and physical exams on new participants and performing periodic reassessments. The Medical Director will develop medical treatment plans and provide medically necessary services in various locations. They will interview and evaluate applicants and their caregivers and participate in the Intake and Assessment decision. The Medical Director will discuss each participant's medical conditions and prognosis with them and their caregivers. They will aid participants in expressing their wishes and planning their future care.

This role requires collaboration with the nursing treatment program of participants. As a member of the leadership team, the Medical Director will contribute to achieving operational goals. Participation as an active member of the interdisciplinary team is essential. The Medical Director will educate and advise the team regarding diagnoses, cognitive and physical function, and treatment options. Participation in team and family conferences, decision-making processes, and educating participants and families regarding health maintenance is also required. Supervision of the health care and treatment program developed by the interdisciplinary team is a key responsibility. The Medical Director will perform physical examinations, order and evaluate appropriate laboratory and diagnostic tests, and record results systematically. Participation in the provider on-call rotation is required as necessary.
