At CSR, our Behavioral Health Department (BHD) offers a comprehensive array of high quality, culturally-sensitive mental health services that provide children and families with the services they need for success at home, in school, and in the community.Position OverviewIntensive Care Coordination (ICC) is a service that facilitates care planning and coordination of services for MassHealth youth, with serious emotional disturbances, under the age of 21, and enrolled in MassHealth Standard or CommonHealth who meet the medical necessity criteria. Care planning is driven by the needs of the youth and developed through a Wraparound planning process consistent with Systems of Care philosophy. Intensive Care Coordinator I is the first level in the ICC series.Essential FunctionsComplete all required trainings, including Child/Adolescent Needs Strength (CANS) certificationProvide intensive care coordination to youth/familiesFacilitate the development of a Care Plan Team (CPT)Conduct CANS and risk assessmentsConvene CPT meetingsCoordinate and communicate with the members of the CPT to ensure the implementation of the Individual Care Plan (ICP)Work directly with the youth and family to implement elements of the ICPCoordinate the delivery of available servicesMonitor and review progress toward ICP goals and update the ICP in concert with CPTConduct all necessary assessments in a timely manner and ensure that they are reviewed and approved as required prior to filingMaintain electronic records in an orderly and timely manner.Participate and come prepared to weekly supervision and staff meetingsEnsure medical case consultations are requested as neededWork closely with Family Partners who share mutual casesFollow through with on-call policies and proceduresAttend necessary trainingsEnsure all services are provided in a professional manner, ensuring privacy and safetyMust be able to meet with assigned families in their home and/or in the community and to transport families as needed.Experience in navigating child/family-serving systems and experience advocating for family members who are involved with behavioral health systemsMust have excellent writing and communication skills.Knowledge of referral coordination to community and private/public resources.Available and able to manage own work schedule to meet each family's scheduling needs during business hours (M-F, 8am – 8pm).Accurately record hours worked and leave time (if applicable) in Electronic Health Record and ADP in accordance with company and department policyPerform other related duties as assigned, within your scope of practice or function areaGeneralMust comply with all regulationsAbility to respect and communicate effectively with co-workers and managersAdheres to agency and departmental regulations, policies and proceduresMandatory reporter of actual and suspected abuse and knowledgeable of agency mandatory reporting practices regarding any suspected cases of abuse, mistreatment, or neglectEducation and ExperienceAn Associate's degree or High School Diploma (equivalency) and a minimum of five (5) years of experience working with the target population.Must be certified in the Massachusetts Children's Behavioral Health Initiative CANS.Must be able to meet with assigned families in their home and/or in the community and to transport families as needed.Experience in navigating child/family-serving systems and experience advocating for family members who are involved with behavioral health systemsMust have excellent writing and communication skills.Knowledge of referral coordination to community and private/public resources.Available and able to manage own work schedule to meet each family's scheduling needs during business hours (M-F, 8am – 8pm).Based on the Department of Public Health regulations, all employees must be vaccinated for influenza and Covid-19.Physical RequirementsThis position demands good mental and physical health and must be able to move continuously during work hours.
