PRN Physical Therapist (PT)
Concord Village Rehabilitation and Wellness – Concord, OH
Flexible PRN Opportunity | Skilled Nursing & Long-Term Care | In-House Rehabilitation
Concord Village Rehabilitation and Wellness is seeking a PRN Physical Therapist (PT) to join our rehabilitation team in Concord, Ohio. If you're looking for a flexible opportunity where you can work alongside an experienced interdisciplinary team and make a meaningful impact on residents’ mobility and independence, we’d love to hear from you.
What You’ll Do- Evaluate residents and develop individualized physical therapy plans of care
- Provide skilled PT services to short-term rehabilitation and long-term care residents
- Help residents improve strength, balance, mobility, endurance, and overall function
- Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
- Complete accurate and timely documentation
- Participate in discharge planning and resident/family education
- Maintain compliance with facility, state, federal, and professional standards
- Current Ohio Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- SNF/LTC experience preferred
- Experience with Net Health and PointClickCare (PCC) preferred
- Strong communication and documentation skills
- Dependable, collaborative, and resident-focused approach
- Flexibility to assist with PRN coverage based on facility needs
- Flexible PRN scheduling
- In-house rehabilitation setting
- Supportive interdisciplinary team
- Opportunity to work with both short-term rehab and long-term care residents
- Collaborative environment focused on helping residents achieve their highest level of independence
