Physical Therapist (PT) – Part-Time & Per Diem

Comfort Creek Nursing & Rehab Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house rehabilitation team. We are currently hiring for both Part-Time and Per Diem (PRN) positions. The Physical Therapist will evaluate, develop, and implement individualized treatment plans to help residents restore mobility, improve functional abilities, reduce pain, and achieve their highest possible level of independence.

This role offers an opportunity to make a meaningful impact by helping residents recover following illness, injury, surgery, or hospitalization while enhancing their overall quality of life in our skilled nursing and subacute rehabilitation setting.

• Evaluate residents and develop individualized physical therapy treatment plans based on physician orders, clinical findings, and resident goals.
• Provide skilled physical therapy interventions to improve strength, balance, mobility, endurance, coordination, gait, transfers, and functional independence.
• Assess and treat residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions.
• Monitor resident response to treatment and modify plans of care as appropriate to maximize outcomes.
• Accurately document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries in accordance with facility, state, federal, and payer guidelines.
• Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, social services, dietary services, and physicians to coordinate resident care.
• Educate residents, families, and caregivers regarding therapeutic exercises, mobility strategies, fall prevention, safety awareness, and equipment use.
• Participate in care plan meetings, family conferences, and discharge planning activities as appropriate.
• Recommend and train residents in the use of assistive devices, mobility aids, and adaptive equipment.
• Maintain compliance with Medicare, Medicaid, state, federal, and facility regulations.
• Support a positive, resident-centered rehabilitation environment focused on clinical excellence and quality outcomes.

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the State of Georgia.
• Current CPR certification or willingness to obtain upon hire.
• Strong clinical assessment and treatment skills.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary rehabilitation team.

• Experience in skilled nursing facilities, subacute rehabilitation, long-term care, or post-acute care settings.
• Familiarity with Medicare documentation and reimbursement guidelines.
• Experience treating geriatric and medically complex patient populations.
• Knowledge of evidence-based rehabilitation practices, fall prevention programs, and functional mobility training.
