Position Summary

The Licensed Practical Nurse (LPN) provides compassionate, high-quality nursing care that promotes residents' health, comfort, dignity, and independence. Working collaboratively with the Registered Nurse, physicians, and interdisciplinary care team, the LPN administers medications and treatments, monitors resident conditions, supports individualized care plans, and helps deliver exceptional resident-centered care.

Key Responsibilities

• Administer medications, treatments, and nursing interventions in accordance with physician orders and facility policies.
• Monitor resident conditions, assess changes in health status, and promptly communicate concerns to the Registered Nurse or physician.
• Record vital signs and maintain accurate, timely, and compliant clinical documentation.
• Assist residents with activities of daily living while promoting dignity, comfort, and independence.
• Collaborate with physicians, Registered Nurses, Certified Nursing Assistants, therapists, and the interdisciplinary care team to implement individualized care plans.
• Provide education, emotional support, and compassionate communication to residents and their families.
• Ensure compliance with infection prevention, resident safety, and regulatory standards.
• Respond appropriately to changes in resident condition and emergency situations.
• Participate in quality improvement initiatives, care conferences, and ongoing professional development.
• Foster a positive, respectful, and collaborative environment focused on delivering exceptional resident care.

Required Qualifications

• Current Licensed Practical Nurse (LPN) license in the State of Kentucky.
• Current CPR certification (or ability to obtain within required timeframe).
• Strong knowledge of nursing principles, medication administration, and resident care practices.
• Excellent communication, interpersonal, and organizational skills.
• Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
• Commitment to compassionate, resident-centered care.
• Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
• Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, and assisting residents with mobility and daily care needs.

Preferred Experience

• One (1) or more years of experience in skilled nursing, long-term care, rehabilitation, assisted living, or post-acute care.
• Experience caring for older adults with complex medical needs.
• Knowledge of electronic medical records (EMR) and nursing documentation systems.
• Experience with medication administration, wound care, restorative nursing, or dementia care.
• Strong clinical judgment, time management, and critical thinking skills.
• Passion for providing exceptional care while helping residents achieve their highest quality of life.
