Bilingual Clinical Care Coordinator

Bilingual Clinical Care Coordinator Under the supervision of the Associate Clinical Officer, the bilingual clinical care coordinator facilitates patient care in both brick‑and‑mortar and mobile clinic settings. The coordinator serves as a primary point of contact for mobile clinical support matters and performs both clinical and clerical duties. Key Responsibilities Facilitate patient care responsibilities, including checking schedules, organizing patient flow, greeting and escorting patients, obtaining comprehensive health history and vital signs, and performing required screenings. Maintain accurate documentation in the patient's electronic health record. Obtain lab, X‑ray reports, hospital notes, and referral information; complete forms and requisitions; verify patient demographics; process and schedule STAT and urgent referrals. Perform administrative clerical duties: data entry, filing, copying, faxing, scanning, in‑house mail distribution, meeting minutes, telephone answering, appointment scheduling, travel plans, report preparation, confidential information handling, and office equipment usage. Maintain patient confidentiality and protect PHI. Assist individuals, families, groups, and communities in accessing health insurance, food, housing, quality care, and health information. Support communication with Spanish‑speaking and special populations. Serve as Executive Assistant to the Chief Medical Officer when needed. Ensure culturally and linguistically appropriate services. Represent the company professionally and positively. Set up and take down mobile medical clinics at outreach sites when medical mobile technicians are unavailable. Complete basic health assessments, referrals, transportation arrangements, and follow‑ups for special populations. Participate in mandatory in‑services and drills, staff meetings, and training programs. Adhere to infection control guidelines. Participate in quality improvement activities, including monitoring, measuring, analyzing, and improving program goals and objectives. Perform clinic maintenance tasks: setting up instruments and equipment, ensuring supplies, inventory management, troubleshooting, and preventive maintenance. Prepare exam rooms, assist provider procedures, and accurately document results. Assist in ensuring supplies and inventory are ready for clinics. Keep equipment operating by following instructions, troubleshooting, and calling for repairs. Maintain a safe, secure, and healthy work environment and encourage compliance with policies and procedures. Support regulatory requirements such as Meaningful Use, PCMH, and UDS through documentation and initiative participation. Perform COVID‑19 rapid testing specimen handling, input results into the electronic health record, and communicate abnormal results to medical providers. Clean exam and procedure rooms and document completed tasks according to clinic protocols. Perform additional duties as assigned by management. Minimum Qualifications Professional, patient‑centered interaction with patients. Cooperative teamwork to deliver safe and quality care. Ability to plan, prioritize, and complete tasks with high accuracy. Capability to work under time constraints while maintaining accurate records. Preferred: 2 years of Certified Medical Assistant experience. Knowledge of medical office protocols, procedures, and terminology. Familiarity with basic computer operations and proficiency in Microsoft Excel, Word, and Outlook. Excellent oral and written communication skills. Bilingual in Spanish required. Experience in preceptoring or training clinic or clerical staff. Organized, detail‑oriented, and effective time‑management skills. Travel Ability to travel to all CRCHC locations and assist at all CRCHC special population clinics. Certification & Licensure Certified Medical Assistant. CPR and BLS certification required. Physical Requirements Continuous walking, standing, and moving about. Frequently bending, kneeling, and crouching. Frequently lifting, pushing, or moving objects up to 50 lbs. Repetitive hand and finger movements for typing and/or writing. Talking and hearing required. Possible exposure to hazardous fumes, airborne particles, toxic or caustic chemicals, bodily fluids, etc. #J-18808-Ljbffr
