The Nurse Practitioner or Physician Assistant will also collaborate with the medical director in the care and treatment of patients.Who You Are:A caring, friendly, and patient-focused provider who takes pride in helping others.Kind and empathetic, yet able to be firm, direct, and professional when needed.A strong communicator who works well within a team.Calm under pressure, adaptable, and motivated to deliver excellent care.Family oriented, modern facility with strong interdisciplinary support.Key Responsibilities:Conduct thorough patient assessments, including physical exams and medical history reviews.Order and interpret diagnostic tests such as lab work, X-rays, and other imaging studies.Diagnose medical conditions and develop treatment plans.Management of hypertension, heart failure, ischemic heart disease, arrhythmias, dyslipidemia, and valvular disease.Evidence-based medication management and provide patient education.Participation in inpatient and therapies (depending on state laws and scope of practice)Educate patients and their families on disease prevention, health maintenance, and management of chronic conditions.Offer counseling on lifestyle changes such as diet, exercise, and stress management.Collaborate with physicians, nurses, specialists, and other healthcare professionals to ensure comprehensive patient care.Provide consultation and referrals to specialists, as necessary.Monitor patient progress and adjust treatment plans as needed.Provide ongoing care for patients with chronic illnesses or conditions.Maintain accurate, up-to-date patient records, ensuring compliance with healthcare laws, regulations, and insurance requirements.Document patient interactions and treatment plans clearly and thoroughly.Serves as a member of committees dealing with health care.Other duties as assigned.Required Skills and Abilities:Strong clinical, diagnostic, and communication skills.Ability to work independently and as part of a team.Proficient in electronic health record (EHR) systems.The ability to educate patients in an empathetic and clear manner.Excellent communication and people skills.Strong organizational and time management abilities.Ability to work effectively within a multidisciplinary team.Must be able to pass a background check and drug screening.Physical Requirements:Able to lift 10 pounds for routine performance of essential functions.May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.Education and Experience:Master's or Doctoral degree in Advance Practice Nursing (MSN or DNP).Cardiology experience preferred. #J-18808-Ljbffr McKenzie Health
