In-Home Support Worker

Collaborate with supervisor and individuals to set goals in daily living and social activities Participate in developing and implementing the Individual’s Service Plan (ISP) Support individuals in scheduling and attending necessary appointments Create teaching moments that foster personal finance and social skills Complete accurate documentation and reports as required Ensure individual well-being and manage crises as needed Requirements BA in human services field or High School Diploma/associate degree plus 2 years relevant experience 6 months’ relevant experience Excellent communication skills Strong organizational and time management skills Ability to work as a member of a diverse, multidisciplinary team Must be comfortable working in community settings Empathy and desire to work with adults with developmental disabilities Must have a valid MA State Driver’s License Core Competencies Demonstrates expertise in developing and implementing Individual Service Plans (ISP) while fostering personal finance and social skills for individuals with developmental disabilities.
