The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Requirements APN License RN License NP Certification CDS and DEA ACLS and BLS 2+ years of experience in geriatrics Shift M-F (8:30a-4:30p) 4 clinic and consult days 1 day follow up home visits for high risk patients recently discharged from the hospital Essential Functions Provide comprehensive primary care with a focus on geriatric needs and chronic disease management See a manageable caseload (about 8-12 patients per day), allowing time for thorough visits and care coordination Work collaboratively with the team to support complex patients and promote healthy aging Participate in hospital home visit initiative to help reduce readmissions At RWJBarnabas Health, our market-competitive Total Rewards package provides comprehensive benefits and resources to support our employees physical, emotional, social, and financial health.
