Key Responsibilities

• Assess and support residents: Evaluate residents' psychosocial needs, create individualized care plans, and provide counseling and emotional support to residents and their families.

• Coordinate care: Collaborate with our interdisciplinary team of nurses, therapists, and other staff to ensure comprehensive and holistic care.

• Advocate for residents: Serve as a liaison between residents, their families, and the community to ensure access to necessary resources and services.

• Maintain records: Keep accurate and confidential client records, documenting progress and ensuring compliance with all regulations.

• Crisis intervention: Respond effectively and compassionately to crisis situations.

Qualifications

• Bachelor's degree in Social Work or a related field.

• Relevant licensure (if applicable).

• Prior experience in a long-term care or senior living setting is preferred.

• Strong interpersonal and communication skills.

• A compassionate, patient, and detail-oriented demeanor.

• Ability to work both independently and as part of a team.
