Registered Nurse Case Manager
We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Willowbrook Hospice in Cartersville, Georgia. As a Registered Nurse Case Manager, you will play a vital role in providing exceptional care to our patients and their families.
About the Role
As a Registered Nurse Case Manager, you will be responsible for coordinating patient care and communicating with patients, families, and healthcare providers. You will assess patient needs, develop and implement individualized care plans, and provide ongoing support to patients and their families. Your expertise in hospice care will ensure that our patients receive the highest level of care and support, addressing their physical, emotional, social, and spiritual needs.
Responsibilities
Assess patient needs and develop individualized care plans in collaboration with patients, families, and healthcare providers
Provide ongoing support to patients and their families, addressing their physical, emotional, social, and spiritual needs
Coordinate patient care with healthcare providers and other healthcare professionals
Communicate with patients, families, and healthcare providers regarding patient care and status
Manage patient data and maintain accurate records
Participate in continuous quality improvement initiatives to ensure the highest level of care
Requirements
Current Registered Nurse (RN) license in the state of Georgia
