Overview
Licensed Practical Nurse (LPN) – Long Term Care (LTC) – Malone, NY. Travel temporary assignment starting 09/28/2026 for 16 weeks. Weekly pay $1,897-$1,911 per week. Guaranteed hours: not guaranteed. Shift: 5x8 evenings. Setting: long-term care facility (LTC - Nursing Home).

Responsibilities
- Administer prescribed medications and monitor patient responses under the supervision of an RN, following physician orders and the patient’s care plan.
- Deliver basic nursing care and assist with activities of daily living (ADLs) such as bathing, grooming, dressing, and mobility.
- Cover CNA duties as needed, including assistance with meals, repositioning, and personal care.
- Document care, vital signs, and changes in patient condition accurately; communicate changes to the supervising RN.
- Monitor patient safety and comfort, respond promptly to changes in condition, and escalate as required.
- Collaborate with the interdisciplinary team to support individualized care plans and maintain quality of care and safety for residents.

Requirements
- Active New York LPN license or proof of application with applied date; NY licensing may take 6-8 weeks and should be noted in submission packet.
- AHA BLS certification (Basic Life Support)
- Minimum 1 year of LTC experience
- Ability to perform CNA duties as needed
- Graduate of an approved LPN program
- Reliable transportation required; public transportation is not available in the area
