Conducting Evaluations and Crafting Treatment Plans

• Conduct evaluations for patients in inpatient and outpatient environments, crafting unique treatment plans tailored to individual needs
• Implement physical therapy interventions concentrating on improving mobility, strength, balance, and everyday function
• Collaborate with a variety of healthcare professionals, including nurses, doctors, and case managers, to optimize patient outcomes
• Educate patients and their families about therapeutic objectives, discharge processes, and home exercise regimens
• Thoroughly document assessments, treatments, and patient progress following facility and regulatory standards
• Engage actively in care team meetings and interdisciplinary discussions to provide coordinated patient care

Education and Certifications

• Doctorate in Physical Therapy (DPT) or a similar accredited degree
• State licensure as a Physical Therapist
• CPR certification preferred
