Home Health Physical Therapist

Schedule: 36 hours a week - day shift

Assignment Details: Holiday coverage required.

Requirements:

• Must have current PT licensure in the state.
• Must have a valid driver's license.
• BLS Certified.
• Minimum of 2 years of experience.

Responsibilities:

• Receive and review physician therapy orders for patients.
• Complete comprehensive patient evaluations and establish written treatment plans with goals.
• Provide skilled physical therapy interventions including exercises, mobility training, and pain management.
• Assess home safety and recommend necessary equipment or modifications.
• Monitor patient progress and modify treatment plans as necessary.
• Educate patients and caregivers regarding treatments and safety measures.
• Maintain accurate clinical documentation.
• Collaborate and communicate with healthcare professionals involved in patient care.
• Travel to patient homes and other care settings as needed.
