Key Responsibilities\n

As a Licensed Clinical Social Worker, you will:

• Conduct thorough biop psychosocial evaluations to identify patient needs and challenges, facilitating effective treatment planning.

• Develop personalized treatment plans and implement evidence-based therapeutic interventions, both individually and in group sessions.

• Maintain clear documentation of social work activities and patient progress in compliance with departmental and regulatory standards.

• Provide education to patients and families regarding mental health recommendations, fostering understanding and compliance with treatment protocols.

• Collaborate closely with a diverse team of healthcare professionals to coordinate care and improve patient outcomes.

• Advocate for patients’ needs in the community, enhancing their access to social support and resources.

• Keep up to date with community resources and best practices in social work to ensure the highest level of care is provided.

• Adhere to ethical standards and the NASW Code of Ethics in all interactions.

Patient Population / Client Focus\n

Your focus will primarily be on patients receiving hospice care, many of whom are facing complex psychosocial challenges associated with serious and terminal illnesses. This role requires sensitivity to the emotional and mental health needs of patients and their families during one of the most difficult times in their lives. Your interventions will aim to help families cope with grief, loss, and the transition associated with end-of-life care.

Qualifications\n

To be considered for this position, applicants must possess:

• A Master’s Degree in Social Work from an accredited institution.

• A current Licensed Clinical Social Worker (LCSW) certification from the State Board of Social Work Licensure.

• Strong understanding of the aging process, human development stages, and cultural considerations in mental health care.

Preferred Experience\n

The ideal candidate will have between six months and one year of clinical experience in a health or mental health setting, preferably working with similar populations as those served in hospice care. Experience applying therapeutic modalities in both individual and group therapy settings is also advantageous.

Compensation and Benefits\n

Clear Sky Family & Marriage Counseling PC offers a competitive salary range of USD 85,000 – 100,000 per year, commensurate with experience and qualifications.
