Key Responsibilities\nAs a Licensed Mental Health Counselor, you will have the following responsibilities:

- Develop and implement personalized, integrated treatment plans that address both psychiatric and medical factors impacting patient care. \n
- Facilitate both individual and group therapy sessions, utilizing evidence-based approaches tailored to each patient's unique circumstances. \n
- Collaborate effectively with a diverse team of medical providers to coordinate care, ensuring optimal treatment outcomes for patients. \n
- Provide immediate crisis intervention and support during mental health emergencies, helping patients navigate acute distress. \n
- Educate patients on the intertwining nature of mental and physical health, fostering patient empowerment and self-management strategies. \n
- Maintain accurate and confidential patient records following legal and ethical standards, including comprehensive documentation of assessments, treatment plans, and progress notes. \n
- Actively participate in team meetings and case conferences, contributing to interdisciplinary care planning discussions focused on enhancing patient outcomes. \n
- Stay informed about current research, best practices, and advancements in mental health and integrated care methodologies. \n
Your primary focus will be on individuals with complex needs, including mental health challenges associated with co-occurring medical conditions such as autoimmune disorders, chronic pain, and substance use disorders. You will work closely with patients to tailor approaches that resonate with their specific life situations and medical histories, providing well-rounded, empathetic support to improve their quality of life.
Qualifications\nThe ideal candidate should possess:

- A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution. \n
- Current and valid licensure as an LCSW, LPCC, or LMFT in California. \n
- Strong interpersonal and communication skills, with the ability to establish trust and rapport with diverse patient populations. \n
- Experience in addressing the mental health needs of individuals with medical conditions. \n
- Proficiency with electronic health record (EHR) systems and relevant computer applications. \n
- Capability to maintain professional boundaries, adhere to ethical guidelines, and ensure patient confidentiality at all times. \n
- Team-oriented mindset, enthusiastic about collaborating with interdisciplinary healthcare professionals. \n
While not mandatory, candidates with the following experiences will be given preference:
