RN Care Manager

About This Role Humana/iCare is seeking an RN Care Manager to join the Family Care Partnership program, providing case management and support to adults in the community. This role is ideal for experienced nurses looking to make a positive impact on member health and safety. Highlights Pay: $71,100 - $97,800/year Location: Racine, WI Department/Unit: Family Care Partnership Schedule: Full-time, Monday - Friday, 8:30 am - 5:00 pm CST Travel: Up to 50% throughout Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth, and Sheboygan counties What You'll Do Conduct home and community visits to complete nursing assessments. Complete the nursing portion of the comprehensive assessment. Monitor wellness and health promotion activities based on member assessments. Re-evaluate member health and wellness at appropriate intervals. Perform routine health monitoring activities based on member needs. Build constructive working relationships with members and their families. Provide appropriate assessment of psychiatric, behavioral, or medical problems in crisis situations. Evaluate members' ability to manage medication and medical care. Provide or arrange services that meet members' identified needs. Assist with program, policy, and procedure development.
