At Twin Oaks Health & Rehab, we are dedicated to providing exceptional care to our residents and are seeking a talented Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping our patients achieve their optimal level of physical function and independence.
About the Role:
As a Physical Therapist at Twin Oaks Health & Rehab, you will be responsible for:
Evaluating and treating patients with various physical disabilities and injuries
Developing and implementing individualized treatment plans to achieve patient goals
Providing education and guidance to patients, families, and caregivers on proper therapeutic exercises and techniques
Working collaboratively with other healthcare professionals to ensure comprehensive care
Maintaining accurate and detailed records of patient progress and treatment plans
Responsibilities:
Assess patients' physical abilities and develop treatment plans to improve function and mobility
Provide individualized therapy sessions, including exercises, stretches, and other interventions
Monitor patient progress and make adjustments to treatment plans as needed
Communicate effectively with patients, families, and healthcare professionals to ensure seamless care
Maintain accurate and detailed records of patient progress and treatment plans
