Job Title: Behavioral Health Care Coordinator

General Summary

A behavioral health care coordinator is a specialized team member who helps rural patients navigate mental health and substance use disorder treatment along with bridging the gap between mental health facilities and primary care providers.

Essential Job Functions

• Care Coordination: Develop personalized care plans, coordinate monthly telephonic/telehealth check-ins, and navigate patients through chronic care management (CCM). Help patients find local clinics, mobile outreach units, or regional behavioral health centers.
• System Integration: Connect rural primary care providers with regional psychiatric consultants, emergency services, and community-based support teams. Set up and guide patients through virtual psychiatric consultations and therapy sessions.
• Cultural Competency: Demonstrate a respectful, open, and unbiased approach toward diverse rural clients, families, and high-need groups. 
• Documentation & Compliance: Maintain secure, data-driven patient tracking aligned with state and federal compliance metrics. Create and track personalized mental health and wellness plans alongside primary care doctors.
• Crisis Follow-Up: Reach out to patients after an emergency room visit or crisis intervention to prevent readmission.

• Word processing software : google docs, Microsoft Word, Excel, PowerPoint
• Electronic Mail Software : Email software, Microsoft Outlook
• Database user software : Adobe

Qualifications/Education and Experience

• Education: Bachelor’s degree in social work, psychology, counseling, public health, or a related field. (Some positions allow relevant case management experience to substitute for a degree).
• Experience: 2 to 3+ years of experience working with high-need populations, health navigation, or behavioral health/social services.
• Core Skills: Strong verbal and written communication skills, cultural competency for diverse rural settings, and proficiency with electronic health records (EHR) or care coordination software.
• Requirements: Valid driver’s license, auto insurance, and background clearance, as these roles often involve community outreach or travel within rural regions.

Physical Requirements

• Seeing : Must be able to see well enough to read emails, documents, reports, etc.
• Hearing: Must be able to hear well enough to communicate with other staff, clients, vendors, etc.
• Standing/Walking : Must be able to move about the office, including up and down stairs
• Climbing/stooping/kneeling : Must be able to stoop or kneel to pick up small items, file documents, etc.
• Lifting/pulling/pushing : Minimal lifting required in this position
