Responsibilities

• Assess patients’ psychosocial, emotional, and financial needs .

Qualifications

• Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW) .
• Licensure:
  • State-issued Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
• Experience:
  • 1+ years of experience in case management or hospital social work (preferred).
• Skills:
  • Strong knowledge of healthcare systems, discharge planning, and community resources .
  • Excellent communication, problem-solving, and advocacy skills .
  • Ability to handle complex cases and diverse patient populations .
• Travel Readiness: Willingness to relocate for temporary travel assignments .
