Create individualized therapy plans to improve strength, balance, and mobility. Accurately document treatments, progress, and outcomes in accordance with facility and payer requirements. Collaborate with occupational therapists, speech‑language pathologists, nursing, and interdisciplinary team members to deliver patient‑centered care. Maintain an 85% productivity standard. Provide therapy coverage at both the Wabasso and Franklin facilities as scheduled. Qualifications Current and valid Physical Therapist (PT) license in the state of Minnesota (or eligibility for MN licensure). Graduate of an accredited Physical Therapy program. Previous SNF or geriatric rehab experience preferred. Excellent communication, documentation, and organizational skills. Reliable transportation for travel between sites.
