RN Case Manager — Suggested Responsibilities

1. Manage an assigned patient caseload

• Conduct/assist with Start of Care visits as assigned.
• Perform comprehensive nursing assessments.
• Develop and update the nursing plan of care.
• Make routine and PRN skilled nursing visits.
• Monitor patient progress and response to treatment.
• Identify changes in condition and communicate them promptly to the physician/NP/PA and DON.
• Coordinate services with PT, OT, ST, MSW, HHA and other disciplines.

2. OASIS and clinical documentation

• Complete assigned OASIS assessments accurately and timely.
• Review documentation for completeness before submission.
• Ensure visit notes support the skilled services provided.
• Ensure medication reconciliation is completed and updated.
• Monitor orders, certifications, recertifications, and care-plan changes.
• Follow up on missing or incomplete documentation.

3. Physician orders and plan of care
The RN Case Manager can help manage the clinical workflow around:

• Obtaining orders.
• Following up on unsigned orders.
• Communicating changes in patient condition.
• Updating the plan of care.
• Making sure ordered services are being provided.
• Communicating changes to the DON.

4. Supervision and coordination
Depending on your agency's policies and the RN's qualifications, the RN Case Manager can:

• Coordinate care with field nurses.
• Review issues identified by visiting nurses.
• Help orient new clinical staff.
• Participate in case conferences.
• Assist with clinical education.
• Help identify competency/training needs.
