Outpatient Physical Therapist

This is a 13-week contract position.

Shift: 5x8.0

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsibilities include delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective return to community activities, and serving as an educator for patients, families, students, coworkers, and self. Adherence to the organization's Mission Statement, Core Values, Code of Conduct, and professional standards is essential. A strong focus on patient and employee safety is a priority, ensuring compliance with safety policies and procedures.

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document patient care clearly and accurately within specified timelines.
• Provide care under signed, valid physician orders and maintain insurance compliance.
• Participate in student program assignments to provide a positive learning environment.
• Create and pursue an individualized professional development plan.
• Adhere to state practice acts and regulatory agency guidelines.
• Participate in performance improvement initiatives to enhance service quality.

• Degree in Physical Therapy from an accredited school.
• 6 months of outpatient clinical experience.
• CPR certification.
• Active and applicable state licensure for practicing as a Physical Therapist.
