As a social worker you will provide comprehensive support across various medical social work areas, including hospital inpatient and transitional care. This position serves as a critical resource within multidisciplinary teams, focusing on patient-centered care and collaboration to ensure optimal discharge planning and coordination of services. Key Responsibilities Collaborate with multidisciplinary teams to assess and support the needs of the patients and their families Coordinate patient discharge planning, ensuring all necessary resources and support systems are in place Conduct biopsychosocial assessments to evaluate patients' needs and appropriate interventions Engage patients and families in discharge planning processes Facilitate communication and coordination with community partners and resources to support ongoing patient care *This position is not eligible for visa sponsorship with the exception of the TN visa classification; Also, Mayo Clinic DOES NOT participate in the F-1 STEM OPT extension program. #J-18808-Ljbffr Mayo Clinic
