Your day as a PCHH RN Case Manager-Sedalia As a new PCHH RN Case Manager at Katy Trail Community Health, you can expect to provide individualized patient care plans, coordinate services, and collaborate with a multidisciplinary team to ensure the best outcomes. What you need to be successful To excel as a PCHH RN Case Manager at Katy Trail Community Health, you must maintain a valid RN licensure in Missouri, showcasing your commitment to professional standards. While experience in a community health center is preferred, your ability to adapt and learn quickly is essential. Additionally, possessing current CPR certification is crucial to ensure you can respond effectively in emergency situations.
