Social Worker

Join our team at Avalon Rehabilitation and Healthcare Center as a licensed Social Worker Rate Range: $25/hr. – $40/hr. (All Inclusive based on experience) Must have SNF/LTC experience. Proudly supported by Marquis Health Consulting Services. Responsibilities of a Social Worker Evaluate patients (existing, new, and prospective) based on their needs and limitations. Address patient concerns and goals while maintaining constant communication with the patient and the case manager. Build rapport with patients and their families. Coordinate care with PT departments. Develop care plans for patients. Qualifications of Social Worker MUST BE Licensed Bachelor’s degree required. A master’s degree is strongly preferred. Experience as a social worker in a healthcare-related setting. Experience within a nursing home or rehabilitation center is strongly preferred. Knowledge of the rules, regulations, and guidelines that govern nursing care facilities. Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
