Rehabilitation & Reentry Case Manager

You will work with Continuum of Care, coordinate GEOtrack records, and support transition planning and rehabilitation programs. You will train, meet inmates, address housing and program needs, prepare reports, and collaborate with staff to ensure safety and smooth operations.
