**About the Role:**

As a CNA, you will work closely with our interdisciplinary team of healthcare professionals to provide personalized care to our residents, ensuring their physical, emotional, and social needs are met. Your responsibilities will include:

* Assisting residents with daily living activities, such as bathing, dressing, and grooming
* Providing vital signs and reporting changes to the nursing team
* Assisting with medication administration and treatments
* Maintaining a safe and clean environment for residents
* Communicating effectively with the care team to ensure seamless care transitions
* Providing emotional support and companionship to residents and their families

**Responsibilities:**

* Provide high-quality patient care in accordance with our facility's policies and procedures
* Collaborate with the care team to develop and implement individualized care plans
* Participate in routine care conferences and contribute to the development of care plans
* Maintain accurate and timely documentation of care provided
* Assist with resident transfers, lifts, and ambulations as needed
* Respond to resident needs and concerns in a timely and professional manner

**Requirements:**
